I understand that all boxes sold to dsmedicalsurplus.com must arrive unopened and undamaged. The term “damage” extends to any box that is crushed, bent, torn, stained or otherwise soiled. Any boxes that is damaged is priced at 50% less then full price shown on price list . Further more All items 8-11 months from expiration will be priced 25% less then full price shown on price list , All items 5-7 months from expiration will be priced 35% less then full price shown on price list. All items 2-4 months from expiration will be priced 50% less then full price shown on price list.
dsmedicalsurplus.com will remove any and all prescription labels before reselling my boxes. I understand that tears caused by the removal of labels are considered damage, and that such boxes will not be accepted by dsmedicalsurplus.com .
It is my responsibility to ensure that my supplies are packaged carefully and securely. dsmedicalsurplus.com will not accept boxes damaged during shipment.
Boxes with broken or missing seals are considered opened, and will not be accepted by dsmedicalsurplus.com .
All items sold to dsmedicalsurplus.com must have a minimum expiration date of (6) months or later from the date of shipment. dsmedicalsurplus.com will not accept boxes that expire in less (2) months from the ship date.
If I send supplies that have less than 2 months until expiration, that have been tampered with, opened, damaged, or otherwise compromised, I understand my supplies will be donated on my behalf.
I understand that the standard delivery time for Business Checks is three days, but may take as long as two weeks depending on USPS delivery speed.
I am the rightful owner of the items being sold to dsmedicalsurplus.com , or I have been granted legal permission to sell these items on behalf of the rightful owner.
I am the rightful owner or have been given permission by the rightful owner to sell these supplies. Furthermore, I signify that I did not, am not and will not violate any applicable law or regulation in obtaining these supplies, nor in selling them to dsmedicalsurplus.com . I represent and warrant that my sale and transfer of these supplies to dsmedicalsurplus.com will not cause dsmedicalsurplus.com to, or result in, dsmedicalsurplus.com violating any anti-money laundering, anti-terrorism, healthcare, or any other applicable law.
The diabetic products I am selling to dsmedicalsurplus.com have not been lost or stolen and legally belong to me and have not been procured by illegal means. The diabetic items I sell to dsmedicalsurplus.com are lawfully owned or possessed by me. In the event that any items I have sold to dsmedicalsurplus.com are found to have been lost or stolen, dsmedicalsurplus.com is under no obligation whatsoever to accept these items, nor compensate me for them in any way whatsoever to include any and all shipping costs.
dsmedicalsurplus.com does not purchase supplies that have been subsidized by any government or insurance program, including but not limited to Medicare and Medicaid.

I understand that shipping times vary and while most packages are delivered within a week, some packages may take up to 14 days.
